Candidates must provide the following:

A completed application processed online using the Mott Foundation application form    March 15
     
The following must be mailed to the Mott Foundation address listed below:
  • An official transcript from the University attesting to academic performance
    OR
    an official completion certification and academic performance document from your high school
    (whichever is applicable)
  • ISEE certification attesting the tax liability of the family for the current year
  •   March 15
